Pravida Health, LLC (“Pravida Health,” “we,” “our,” or “us”) operates the website located at pravida.com (the “Site”) and provides medical and wellness services from our office at 1801 Peachtree St NE, Ste 150, Atlanta, GA 30309. This Privacy Policy explains how we collect, use, disclose, and safeguard the personal information you provide to us through the Site and through your interactions with our practice. Please read this policy carefully. By using the Site or engaging our services, you agree to the practices described here.
This Privacy Policy applies only to information collected through the Site and through our administrative and marketing communications. Protected Health Information (“PHI”) that we create, receive, maintain, or transmit in connection with providing you with treatment, payment, or health care operations is separately governed by our HIPAA Notice of Privacy Practices, which you may request at any time by contacting our office.
1. Information we collect
Information you provide directly. We collect information you submit through contact forms, appointment requests, membership inquiries, chat interactions, and other communications. This may include your name, email address, phone number, mailing address, date of birth, insurance details, and any health-related questions or context you choose to share.
Information collected automatically. When you visit the Site we collect certain information automatically through cookies, pixels, and similar technologies. This includes your IP address, device type, browser type, referring URL, pages viewed, time spent on pages, and other standard analytics data. We use this information to understand how visitors use the Site, to improve content, and to measure the effectiveness of our marketing.
Information from third parties. We may receive information about you from third parties, including our patient portal provider, our electronic health record vendor, our practice management platform, our email and marketing platforms, and our advertising partners. We may also receive information from health plans, referring physicians, laboratories, and diagnostic imaging providers when they are involved in your care.
2. Protected Health Information
Once you become a patient of Pravida Health, the health information we create or receive about you in the course of providing care is Protected Health Information under HIPAA. Our use and disclosure of PHI is governed by federal and state law and is described in our HIPAA Notice of Privacy Practices, not by this Privacy Policy. If any provision of this Privacy Policy is inconsistent with our HIPAA Notice of Privacy Practices with respect to PHI, the HIPAA Notice controls.
3. How we use your information
We use the personal information we collect to:
- Respond to inquiries, appointment requests, and communications you initiate
- Deliver, personalize, and improve the Site and our services
- Schedule, confirm, and remind you of appointments
- Process membership and service payments
- Send you administrative messages, newsletters, and health education content you have requested

4. How we share information
We do not sell your personal information. We share information only in the following limited circumstances:
Service providers. These vendors are contractually required to protect your information and use it only for the services they provide to us. Where PHI is involved, these vendors execute Business Associate Agreements under HIPAA.
Referrals and continuity of care. With your authorization, we may share information with other physicians, laboratories, imaging centers, pharmacies, or specialists involved in your care.
Legal obligations. We may disclose information when required by law, subpoena, court order, or governmental request, or when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.
Business transfers. If Pravida Health is involved in a merger, acquisition, financing, reorganization, sale of assets, or similar transaction, information may be transferred as part of that transaction. We will notify you of any change in ownership or use of your personal information.
5. Cookies and tracking technologies
The Site uses cookies, web beacons, pixels, and similar technologies to remember your preferences, analyze site usage, and support marketing. You can control cookies through your browser settings. Blocking cookies may affect the functionality of certain parts of the Site. We honor Global Privacy Control signals where required by applicable law.
6. Third-party analytics and advertising
We use third-party analytics services (such as Google Analytics) to help us understand how visitors use the Site. Medical records are retained in accordance with Georgia state law and federal HIPAA requirements. Non-medical inquiry information is typically retained for up to three years after your last interaction with us unless a longer retention period is required by law.
9. Your rights and choices
Access, correction, deletion. Subject to applicable law, you may request access to the personal information we hold about you, request that we correct inaccurate information, or request that we delete information that is no longer needed. Some information (including medical records) may be subject to legal retention requirements that limit our ability to delete it.
Marketing preferences. You may opt out of marketing emails at any time by using the unsubscribe link in any marketing message or by contacting us directly. Even if you opt out of marketing communications, we may still send you administrative messages relating to your care or account.
State-specific rights. Residents of California, Colorado, Connecticut, Virginia, Utah, and other states with applicable privacy laws may have additional rights, including the right to know what personal information we collect, the right to correct or delete it, the right to opt out of the sale or sharing of personal information (we do not sell your personal information), and the right to non-discrimination for exercising these rights. 10. Children’s privacy
The Site is not directed to children under 13 and we do not knowingly collect personal information from children under 13 through the Site. If you believe we have collected information from a child under 13, please contact us and we will delete it. Care for minor patients is coordinated directly with a parent or legal guardian and is subject to applicable state consent laws.
11. Third-party links
The Site may contain links to third-party websites, including our patient portal, social media platforms, published research articles, and other external resources. We are not responsible for the privacy practices of those websites. We encourage you to review the privacy policies of any third-party site you visit.
12. Changes to this Privacy Policy
We may update this Privacy Policy from time to time. When we do, we will update the “Last updated” date at the top of this page. Material changes will be communicated more prominently on the Site or via direct notice. Your continued use of the Site after the effective date of the updated policy constitutes acceptance of the changes.
